Department of Health (DOH) Undersecretary Dr. Myrna C. Cabotaje requested the Commission on Higher Education (CHED) to have their teaching and non-teaching personnel registered after the CHED requested the DOH include its personnel in the Priority Eligible GroupB for COVID-19 vaccination Program of the national Government.
All CHED Regional Offices are tasked to contact the higher education institutions (HEIs) under their jurisdiction and survey the names of the personnel who are willing to join the COVID-19 Vaccination Program. This registry will be forwarded to the DOH Sub-Task Group on Registry and Data Management for consideration.
Registration will be done through the following Google form links below. Teaching and non-teaching personnel will have two separate forms assigned. The links are as follows:
